Retrospective Case Series Analysis: Clinical and Functional Outcomes of Dacryocystorhinostomy Surgery

Abstract: Background: Dacryocystorhinostomy (DCR) is the gold-standard surgical treatment for nasolacrimal duct obstruction (NLDO), creating a direct passage between the lacrimal sac and nasal cavity to restore tear drainage. Although success rates are high, both anatomical and functional factors can contribute to surgical failure. This five-year retrospective study reviews 50 DCR cases to assess patient demographics, causes of failure, associated intranasal pathologies, and outcomes of revision procedures performed with silicone intubation. Methods: A retrospective analysis of 50 consecutive DCR surgeries was conducted at a tertiary care centre. Data on demographics, surgical approach, laterality, and reasons for failure were collected. Intranasal findings, including deviated nasal septum (DNS) and adhesions, were documented. Outcomes of revision surgeries using silicone intubation and adjunct Mitomycin C (MMC) were also evaluated. Both adult and paediatric patients were included. Results: Of the 50 patients, 45 were adults and 5 paediatric. Thirty-three were female, and 17 males. Left-sided surgery predominated, and all cases involved silicone intubation. The failure rate was 10%. DNS accounted for three failures, intranasal adhesions for one, and one required nasolacrimal probing. Revision DCR with MMC resulted in successful outcomes, with paediatric cases showing good recovery. Conclusion: Detailed nasal evaluation and ENT collaboration are essential for reducing DCR failure. Silicone intubation and MMC enhance revision success.
