CT-Based Splenic Volumetry as a Non-Invasive Imaging Marker of Portal Hypertension
Abstract: Background: Portal hypertension is characterized by a spectrum of imaging findings on contrast-enhanced computed tomography (CT), including splenomegaly, portal vein dilatation, collateral vessels, gastroesophageal varices, and ascites. CT-derived splenic volumetry provides an objective method for quantifying splenic enlargement; however, its role in routine assessment of portal hypertension requires further evaluation. Objective: To describe CT-derived splenic volume categories and accompanying CT manifestations of portal hypertension in adults undergoing contrast-enhanced abdominal CT. Methods: This prospective cross-sectional study was conducted in the Department of Radiodiagnosis at City Heart Superspeciality Hospital, Hamirpur, Himachal Pradesh, from Jan-2026 to June-2026. Eighty consecutive adult patients with portal hypertension who underwent contrast-enhanced abdominal CT were included. Splenic volume was measured using a standardized CT volumetric technique on a dedicated workstation. CT findings including portal vein dilatation, splenic vein enlargement, collateral vessels, gastroesophageal varices, ascites, hepatic morphological changes, recanalized paraumbilical vein, retroperitoneal collaterals, and portal vein thrombosis were recorded. Data were analyzed using descriptive statistics. Results: The study included 80 patients, comprising 56 males (70.0%) and 24 females (30.0%). The largest proportion of patients belonged to the 41?50-year age group (35.0%). Splenic enlargement of varying degrees was observed in most patients, whereas 10 (12.5%) had splenic volumes within the predefined normal range. Increased splenic volume frequently coexisted with portal vein dilatation, splenic vein enlargement, collateral vessels, gastroesophageal varices, ascites, and hepatic morphological changes on contrast-enhanced CT. Conclusion: CT-derived splenic volumetry provided an objective measure of splenic enlargement in this cohort. Increased splenic volume frequently coexisted with portal vein dilatation, collateral vessels, gastroesophageal varices, and ascites. Further patient-level statistical analysis using reproducibility testing and an established reference standard is required to determine its independent diagnostic and prognostic value.
