Abstract: Background Tenofovir has been recently introduced in our country as first line therapy in HIV infection but limited data available on safety profile & tolerability in Indian population of patients. the study will focus on toxicity, particularly nephrotoxicity & tolerability of Tenofovir. Aims of the studyTo study the toxicity and tolerability of tenofovir in tenofovir based first line anti retroviral therapy in ART nave patient. Study type and Design- Descriptive & Longitudinal Study. Place & duration of the study- The ART center in North Bengal Medical College & Hospital, Darjeeling, WB, From July 2014 to June 2015. Study Population- We selected 107 population, all ART nave patients (aged 18years and more) attending ART center, North Bengal Medical College & Hospital who will be started on Tenofovir based 1st line ART ( according to NACO guideline) except Pregnant patient, patients with serum creatinine >1.2mg/dl & unwilling for consent. Discussion The study showed that there is increasing value of mean serum creatinine level of total study population from base line value but mean serum creatinine at the end of study remained within normal reference value. The study population had overall weight gain at the end of the study. Results This study shows that tenofovir is well tolerated drug. Tenofovir therapy is associated with mean weight gain and increased in haemoglobin level. Conclusion Tenofovir is well tolerate and very safe drug in these patients without prior renal disease and concurrent nephrotoxic drugs, the follow up of these patients should be done to know the exact incidence of nephropathy Nephrotoxic, weight, mean, creatinine Author for communicationDr Kapildev mondal{kapilmondal980@gmail. com}
